At Luma Dermatology, Dr. Metterle and Dr. Patel are committed to providing the highest quality care for all our patients, including those experiencing hair loss. To ensure the most thorough and effective evaluation possible, we have recently updated our office policy regarding hair loss appointments.
What’s Changing?
If you are scheduling a new appointment for hair loss—meaning you have never been seen or evaluated for hair loss at our office before—you will now be required to complete a Hair Loss Questionnaire before your scheduled visit. This comprehensive form allows us to gather critical information about your condition in advance, ensuring that your appointment is as productive and informative as possible.
How to Obtain the Questionnaire
Patients can obtain the questionnaire in one of two ways:
Email Request: You may request the form via email, which allows you to print and complete it at your convenience.
In-Person Pick-Up: You may visit our office to collect a copy of the questionnaire in person before your scheduled appointment.
Please note that we will not be mailing forms due to associated costs and the risk of incomplete follow-through on appointments.
Bringing the Completed Form to Your Appointment
The Hair Loss Questionnaire is extensive and must be fully completed prior to your visit. We ask that you bring the completed form with you to your scheduled appointment. Forms cannot be filled out in the waiting room before your visit, and if you arrive without a completed form, your appointment will need to be rescheduled. The questionnaire requires detailed responses and may necessitate bringing specific medical records, which are essential for a thorough evaluation.
For Patients Discussing Hair Loss During a Different Appointment:
If you are already scheduled for another type of dermatology visit (such as a skin check or a consultation for a different concern) and bring up hair loss as an additional issue, we kindly ask that you schedule a separate dedicated hair loss appointment. Our team will provide you with the Hair Loss Questionnaire during your visit, and you can book your follow-up appointment at checkout. This ensures we allocate the appropriate time and resources to thoroughly assess and address your hair loss concerns.
